Who is eligible?

To be eligible for this State program, individuals must:

  • Need consistent support with everyday tasks like bathing, dressing, and meals
  • Receive Medicaid benefits
  • Have the capacity to self-direct care or have a capable adult oversee program obligations on their behalf

Fiscal Intermediary Services: As a Fiscal Intermediary (FI), Health Force guides you and your designated representative through the process, ensuring seamless payment to your personal assistants.

Partnerships: Health Force collaborates with numerous managed care insurance organizations, managed long-term care companies, and local departments of Social Services to provide comprehensive support.

Who can be a Personal Assistant?

To qualify as a Personal Assistant, individuals must meet the following criteria:

  • Age Requirement: Must be at least 18 years old.
  • Relationship Limitation: Cannot be the Consumer’s spouse, parent, or designated representative, except if the Consumer is over 21 and the parent is not responsible for them.
  • Work Authorization: Must have the legal right to work in the United States and provide appropriate documentation.
  • Interview Process: Must undergo an interview with the Consumer to ensure suitability for providing required care.
  • Medical Requirements: Once selected, the Assistant must fulfill medical prerequisites, including a physical examination and providing immunization records.
  • Medicaid Eligibility: Must be eligible to work for someone whose services are covered by Medicaid.
