Home Care/Community Care Services

Health Force offers a variety of Home Care and Community Care options. Each client's needs are special and we provide diverse options to customize a care plan to fit the needs of each individual. Our services are available from as little as a few hours a day a few days a week to round-the-clock care. The staff works with the client, their family, and health care team to ensure that the needs of the patient are being met to their full satisfaction.

NHTD and TBI Waiver Services

Health Force is proud to be selected to be a provider of the New York State Waiver Services. We participate in the TBI Waiver and the NHTD (Nursing Home Transition Diversion) Waiver program. Both these programs allow Western New York residents the ability to stay in their own home as opposed to living in a nursing home. The Waiver programs also permit HEALTH FORCE to assist in returning a client to the community by locating a suitable apartment, and ensuring all services are in place. Waiver services include a Service Coordinator to arrange for aide services for the length of time required by the client, securing of an apartment and assistance in necessary furnishings, Our aides assist with those daily tasks often taken for granted along with assistance with meal preparation and shopping, escorting the client out into the community and . Also included is counseling and guidance to acclimate the client in their return to the community.

Many health insurance policies require a separate rider to cover home health care services. So it is best to check with your provider.

[1] Health Force is a provider for the New York State Nursing Home Transition and Diversion waiver program. With this program we are able to assist participants to return to their home or their community after a stay in a nursing home OR assist them in remaining in their home as opposed to being placed in a nursing home. We will be able to ensure their safety as well as provide assistance for daily personal care needs.

[2] Health Force is a provider of the New York State Traumatic Brain Injury waiver program. We help participants who have sustained any type of brain injury-whether it is from a stroke or physical injury to the brain. We assist them in staying in the community and maintaining their independence as well as assisting to stay at the optimal functioning level.
